• APPOINTMENTS:
Appointments may be made by calling 972-271-6811 for Garland; 972-548-1145 for McKinney; or call our toll-free number at 1-877-271-6811. Every effort will be made to provide the earliest possible appointment for the convenience of the patient.
If you are unable to keep your appointment, please cancel as far in advance as possible. Some other patient who can be booked into the open time will be grateful for your thoughtfulness.
• PATIENT REGISTRATION FORMS:
For your initial visit, please print and complete the forms below. You may fax the completed forms to our office or bring them with you to your scheduled appointment. (Fax: 972-278-6589)

– Allergy Questionnaire (bring or fax)
– Hives Questionnaire (bring or fax)
[Only complete the Hives Questionnaire if you have had persistent hives for more than 6 weeks]
– Financial Policy (bring or fax)
– Preparation for Allergy Evaluation (for your information)

• WHAT TO BRING TO YOUR APPOINTMENT:
– Your insurance card
– A list of medications, including the dose and frequency
– All completed patient forms (see above)
– Recent test results related to your condition
• EMERGENCIES:
If you are having a severe allergic reaction or an asthma attack, call 911. If it is urgent that you speak to Dr. Page, you can reach him at 972-271-6811 during routine office hours; after hours, you may contact him at 469-831-3635.
• REFERRALS/AUTHORIZATION:
Many patients have insurance companies that require a referral and/or authorization for procedures. If your insurance company requires this, it is your responsibility to make sure we have a CURRENT referral from your Primary Care Physician prior to any visits and CURRENT authorization prior to any procedures being performed.
