If you are unable to keep your appointment, please cancel as far in advance as possible. Some other patient who can be booked into the open time will be grateful for your thoughtfulness.
If Canceling Your Appointment, Please Give a Notice of 24 hours to Avoid a $25 Cancellation Fee.
NEW PATIENT REGISTRATION FORMS:
For your initial visit, please print and complete the forms below. You may fax the completed forms to our office or bring them with you to your scheduled appointment. (Fax: 972-278-6589).
To allow accurate skin testing, stop all antihistamines at least 48 hours prior to your allergy evaluation. Please note: Atarax and Hydroxyzine need to be stopped 1 week before your appointment. (See preparation for allergy evaluation for a list of medications).
Please arrive 30 minutes before your appointment to complete the check in process.

WHAT TO BRING TO YOUR APPOINTMENT:
- Your insurance card
- A list of medications, including the dose and frequency
- All completed patient forms (see above)
- Recent test results related to your condition
- Foods you suspect you are allergic to

Many patients have insurance companies that require a referral and/or authorization for procedures. If your insurance company requires this, it is your responsibility to make sure we have a CURRENT referral from your Primary Care Physician prior to any visits and CURRENT authorization prior to any procedures being performed.
